Animal Use Protocols (AUP) Forms
Animal Use Protocols (AUP) forms are prerequisites for approval of animal-related research and teaching. AUP forms must be completed and approved by the PCAC prior to the commencement of research or teaching.

Animal Procurement Form (386 KB) | To be completed once AUP is approved and before ordering any animals. |
Annual Renewal or Amendment Form (36 KB) |
Animal Use Protocols must be renewed annually or amended when any changes are made to the research (e.g., change in type or number of animals, change in use of animals). |
Evidence of Practical Training Form (36 KB) | To be completed by all investigators and any research personnel attached to the protocol. |
Animal Adoption Release and Waiver Form (37 KB) | To be completed by anyone adopting out a laboratory animal. |
AUP Closure Form for Completed Study (80 KB) | To be completed by principal investigator after a project is completed. |
