This form will allow you to request a change of your uregina account username and/or staff email address if you have legally changed your name.

Please, notify Human Resources (Staff/Faculty) or the Registrar's Office (Student) of your name change BEFORE you proceed to complete this form.

If you have Not changed your name, but wish to request a new username or employee email address, you can proceed with submitting this form. Note that username changes are only approved in very rare circumstances. Please include the reasons for your request in the 'Message' section below.
