Proxy Registration Form (23k) - used if a student wishes to give permission to another person to register on their behalf
Undergraduate Audit Registration Form (145k) - used if a student wishes to attend lectures of a course, but not enrol in the course for credit, and otherwise participate in the class only to the extent permitted by the instructor.

Registration Permit/Override (39k) - used when a student wants to request permission to override a restriction for a class (pre-requisite waiver, college restrictions, etc)
Application for Admission for Visiting Students - Visiting Students are from a recognized post-secondary institution who are taking courses for credit at the University of Regina towards a program at their home institution. Students who would previously have used the Common Visiting Student Application Form now use the Application for Admission for Visiting Students, effective August 30, 2023.
Request for Credit by Challenge Exam (24k) - Challenge Examinations allow a student to obtain credit by demonstrating mastery of the course material in a comprehensive examination.
Seniors' Tuition Waiver Request Form (70k) - Request form for students aged 65 years or older who may qualify for a tuition waiver. See also: Seniors' Tuition Waiver FAQ.

Convocation and Graduation Forms

NOTE: To facilitate Convocation preparations, request forms for duplicate, replacement, or early release of parchments will close on September 15 (Fall Convocation) and May 15 (Spring Convocation) and will be available again the Monday following Convocation. Order Duplicate, Replacement, or Early Release of Parchment through the Registrar's Office online store.

Student Records

Change of Legal Name (140k) - a student who provides supporting documentation may change their name on University records. See Policy EMP-100-005 Legal Name and Name Change for more information.
Update Name and/or Gender - login to UR Self-Service using your ID and password. Click on Student, then Personal Information, then Update Name and/or Gender and complete the form.
Self-Declaration of Indigenous Status - follow the instructions to self-declare
Request for Documents - fill out this online form to request a document from your student file
Third Party Information Release of Student Information (53k)

Health Related and Other Special Forms

U of R Student Health Certificate (81k) - used when either an application of adjustment of tuition and/or grades, or an application for deferral of term work and/or final exam requires medical documentation
Application for Tuition Adjustment (140k) - a student should complete this form AFTER a withdrawal from classes for reasons beyond their control (such as illness, accident, or involuntary job transfer) after the end of the 100% refund period.
Application for Grade Adjustment (331k) - students who become ill or suffer an accident or bereavement after the published academic withdrawal deadline, or unsuccessfully attempt to continue in classes after the deadline, should consult their faculty, federated college, or academic unit about the options available to them.
Application for Deferral of Term Work and/or Final Exam (Undergraduate Student) (146k) - Undergraduate students should complete this form to request an extension of deadlines for completion of assignments or final exams due to illness, death in the family, or other extreme and legitimate circumstances beyond their control.
Religious or Cultural Accomodation (964k) - Students who require religious or cultural accomodations may submit this form.
Graduate Studies student must fill out the deferral form in the Student Request Forms section of Graduate Studies and Research .
Self-Declaration of Illness (138k) - Submit this form to your faculty, federated college, or academic unit office if you are unable to complete course work and/or exams due to illness.
