Safety Training Registration Form

You must have successfully completed the online training for Chemical & Lab Safety (CLS) and/or Biosafety Awareness to be eligible to request a mandatory workshop.  You must have attended a CLS mandatory workshop and successfully completed the CLS WHMIS exam PRIOR to request a Biosafety Awareness workshop.  No exceptions!

NOTE:   Participants will have three weeks from the online course registration date (Chemical & Lab Safety and/or Biosafety Awareness) to successfully complete the online training AND successfully complete a Chemical & Lab Safety mandatory workshop and obtain a passing grade of 80% on the CLS WHMIS exam.

Participants who require a mandatory Biosafety Awareness workshop, will have three weeks from the Chemical & Lab Safety workshop date to attend and successfully complete a Biosafety Awareness workshop and successfully complete, within one week, the Autoclave and Hazardous Identification assignments.
