Reporting an Incident

Faculty and Staff

Report any incident that involves injury to a person or damage to property (or had the potential to do so) to Health, Safety & Wellness, Human Resources, within 24 hours of occurrence.  After completing and signing page 1 of the Incident Report form, please provide the Incident Report (Page 1 & 2) to your supervisor immediately.

After your supervisor has reviewed and signed page 2 of the Incident Report, please submit BOTH pages to or stop by the Human Resources office (AH 435) and drop it off at the front counter.
