Reporting an Incident

Instruction for completion of incident forms


Faculty or Staff:

  • Please report any incident that involves injury to a person or damage to property, or had the potential to do so, to Health, Safety & Wellness, Human Resources, within 24 hours of occurrence.
  • Your job is to complete an incident report form.
  • After completion, sign and give the form to your supervisor immediately.

Completed incident reporting forms can be submitted to:
